The Center is located in the city's heart at 130 N. Broadway, Suite 300. Since opening in 2013, Community Health’s Shawnee Family Medical Center has experienced more than 28,000 patient visits by Pottawatomie County residents seeking an affordable, accessible primary care medicine home for all their healthcare needs.

Shawnee Family Medical Center significantly improves health outcomes for a large, previously underserved population in Pottawatomie County. Most Center patients are from low-income households, lack health insurance, and qualify for Medicaid.

The Center, with a staff of seven, in addition to two nurse practitioners and one physician, fills a critical community need by offering the following services: primary medical care (diagnoses, treatments, and medications for temporary and chronic illnesses); behavioral health (addiction and mental health); and case management. 

 In 2021, the Center welcomed 1,441 individual patients, that totaled 4,431 visits. Patients ranged in age from under 1 to over 85, and women represented 62 percent of all patients served.
